Successful treatment of postoperative massive pulmonary embolism with paradoxal arterial embolism through extracorporeal life support and thrombolysis

چکیده
Pulmonary embolism is a common clinical entity related to high mortality. About 200,000 to 300,000 patients die every year due to pulmonary embolism. The purpose of this article is to describe a case of a patient who on the second postoperative day after undergoing thromboembolectomy of the left femoral artery, manifested a massive pulmonary embolism. Due to cardiorespiratory collapse a combined treatment via extracorporeal life support (ECLS) and parallel catheter thrombolysis was decided and performed. By cardiorespiratory improvement and final stabilization the patient was successfully weaned from ECLS and the system was successfully removed. After a reasonable postoperative time the patient was dismissed in good overall condition.

متن کاملMassive pulmonary embolism requiring extracorporeal life support treated with catheter-based interventions.
When pulmonary embolism (PE) develops, circulatory collapse and hypoxia are caused at the same time. The rapid and proper use of extracorporeal life support (ECLS) can improve the mortality rate of patients with collapsed massive PE. No study has examined the influence of treatment that involved adding catheter based-intervention to ECLS with massive collapsed PE. Thirty-five patients with mass...
متن کاملPrognostic Impact of Thrombolysis in Myocardial Infarction Risk Index on Hospitalization Mortality of Patient with Acute Pulmonary Embolism
Introduction: Acute pulmonary embolism (PE) is one of the deadly cardiovascular diseases. One of the indexes proposed in these patients for risk stratification is the Thrombolysis in Myocardial Infarction (TIMI) risk index (TRI), which includes three parameters of systolic blood pressure, age, and heart rate. This study aimed to evaluate the predictive value of TRI on in-hospit...

for Acute Pulmonary Embolism To the Editor: We read with great interest a review paper titled “Acute Pulmonary Embolism, Part II: Risk Stratification, Treatment, and Prevention” by Goldhaber and associates in the December 2003 issue of the journal.1 The strategies for acute pulmonary embolism in their review are anticoagulation, inferior vena caval filters, thrombolysis, and embolectomy. They m...

Venous thrombo-embolism is a life threatening condition with often non specific presentation. The detection of massive pulmonary embolism in the intra and immediate post-operative period is not only difficult due to the variety of conditions with similar presentation, but the therapy for the same is complicated with concerns of surgical and intracranial bleeding precluding various options. We p...
